PM-JAY Hospital Fraud: Were You Charged Cash on a Cashless Admission?

If you have an Ayushman Bharat (PM-JAY) card and were admitted to an empanelled hospital for a covered procedure, you should pay zero cash. Any cash demand on a PM-JAY cashless admission is illegal. Full stop.

Below: the five ways this rule gets broken, real cases where the government caught it, and what you can do right now.

The rule is simple

PM-JAY card + empanelled hospital + covered procedure = you pay nothing. The hospital gets paid directly by the government at a fixed package rate. Cash on top of that is a violation.

The scheme covers about 55 crore people (550 million) across 12.37 crore families, according to the Health Minister's Rajya Sabha reply in March 2025. On 29 October 2024, coverage expanded to all senior citizens aged 70 and above — roughly 6 crore more people.

1. You paid cash — but the government also paid

This is the most common pattern. The hospital admits you under PM-JAY, files a claim with the government, and still asks for cash at discharge. You pay once. The government pays again. The hospital collects twice.

In Chhattisgarh, SMC Heart Institute charged a family ₹90,000 for a heart procedure (ventricular septal defect closure) while also processing a PM-JAY claim of ₹1.1 lakh for the same surgery. The hospital was penalised when the state cracked down on 23 private hospitals. (ThePrint, April 2025.)

In Meghalaya, a CAG audit found that out of 19,459 patients treated at 5 private hospitals between February 2019 and March 2021, 13,418 — that is 69% — had to pay cash at discharge despite holding Ayushman cards. Total amount: ₹12.34 crore. (CAG Performance Audit on AB-PMJAY, presented in Parliament, August 2023.)

2. You never went — but a claim was filed in your name

Did you actually visit the hospital? In ghost billing, the hospital files a claim for a patient who was never admitted, or a procedure that never happened. The government pays. The patient never knew.

In Uttar Pradesh, someone hacked or misused portal credentials to process 6,239 claims from 39 hospitals — ₹9.94 crore in total. The claims went through late at night, between 1 and 22 May 2025. An FIR was filed at the Hazratganj cyber cell. (Times of India, February 2026; Medical Dialogues.)

3. A "free health camp" sent you for surgery you did not need

Someone sets up a camp in your village. Free check-up. They find your Ayushman card. Next thing you know, you are in a hospital getting a procedure. The hospital claims the package rate from the government. You may not have needed the surgery at all.

In Ahmedabad, Khyati Multispecialty Hospital ran a camp in Borisana village (Mehsana district). Nineteen villagers were brought in. Seven got angioplasty. Nine people died. The cardiologist was arrested — charges include culpable homicide, forgery, and PM-JAY fraud. The CEO and four others were also arrested. Across India, 56,217 surgical fraud cases have been flagged under PM-JAY, according to a Health Ministry reply in Parliament (December 2024).

4. The lab reports were changed to justify the claim

A hospital can alter test results or X-rays to make a procedure look necessary — or to claim a higher-value package. You might never know, because the "evidence" for your surgery was fabricated.

Nihit Babycare Children Hospital in Rajkot tampered with lab reports and X-rays of 116 children to claim PM-JAY money. The fine: ₹6.54 crore — ten times the ₹65.47 lakh claimed. The hospital was removed from PM-JAY and the doctor's license was suspended. (Times of India, 29 June 2024; Indian Express, November 2024.)

5. Claims filed for impossible dates — or after the patient died

NHA's system catches some of the most brazen attempts: claims with dates that don't exist, or claims for patients who had already passed away. The Health Minister told the Rajya Sabha in March 2025 that 3.56 lakh such claims — worth ₹643 crore — have been rejected.

How to check — four questions

  1. Did you pay cash? Any money you paid during a PM-JAY admission for a covered procedure is a violation. Keep every receipt — even handwritten ones.
  2. Were you actually there? Open the Ayushman app or go to beneficiary.nha.gov.in. See what claims exist against your card. If one shows a hospital you never visited, that is ghost billing.
  3. Did you need the procedure? Were you feeling fine before a "free camp" sent you for surgery? Ask for your medical records. Get a second opinion from a government hospital.
  4. Do the dates make sense? Check if admission and discharge dates on your claim match when you actually went.

What to do right now

  1. Call 14555. Free. National. This is the Ayushman Bharat anti-fraud helpline.
  2. File online. Go to pmjay.gov.in → grievance section. Or use the Ayushman app.
  3. Write to the District Collector or CMHO. If you paid cash, put it on paper. Attach receipt copies. A written complaint creates a record.
  4. Still inside the hospital? Ask for the Ayushman Mitra. Every empanelled hospital must have one — their job is to handle PM-JAY cases.

Can a PM-JAY hospital charge me cash?

No. Any cash demand on a covered procedure at an empanelled hospital is illegal under PM-JAY rules. If you were charged, call 14555 and file a written complaint with the District Collector or CMHO.

How do I check if a claim was filed in my name?

Use the Ayushman Bharat app or visit beneficiary.nha.gov.in. You can see all claims processed against your Ayushman card. Look for procedures you do not recognise or dates when you were not in any hospital.

What happens to hospitals caught committing PM-JAY fraud?

De-empanelment (2,003 hospitals in FY2025-26), monetary penalties (₹114 crore in FY2025-26), and FIRs in serious cases. In the Khyati Hospital case (Ahmedabad, 2024), the cardiologist was arrested and charged with culpable homicide after 9 patient deaths from unnecessary stent procedures.
